Amendment/Notification Forms

Amendment and notification forms are only to be used by charter schools sponsored by the Arizona State Board for Charter Schools and the Arizona State Board of Education that have a signed charter contract. All amendment and notification forms must be submitted, approved and signed by both parties before the school can take action on or implement the changes requested.  Please send the signed original form to our office for processing.
